Pricing and Recommended Scheduling: ________

I acknowledge that grooming prices are based on my pet's breed, size, hair type and coat condition as determined by the groomer's assessment. Pricing is reflective of a regular grooming schedule of every 5-8 weeks, and extended gaps between appointments may result in increased pricing due to the length and condition of the coat. To help manage costs effectively and maintain my pet's coat in optimal condition, I understand that a consistent grooming schedule is recommended. Additionally, the Face, Feet, and Fanny service is available as an in-between maintenance option but is not a substitute for a full groom. To ensure my pet's coat remains in good condition, I agree to alternate between full grooming sessions and maintenance trims as needed. For more details on pricing, I will refer to the updated service menu.

Drop off timing vs. start time of service: ________

I understand that the drop-off time for my pet does not necessarily indicate the start time of the grooming service, as appointments are staggered to ensure each pet receives the full attention and care they deserve. If I have time constraints, I will inform the staff at drop-off, and they will do their best to accommodate my request. However, I acknowledge that a guaranteed completion time cannot be promised. If your pet needs to stay for additional boarding beyond one hour after grooming is completed, and I haven't communicated this in advance, an additional boarding fee may apply. With this in mind, if you have a day filled with time restraints, we recommend booking on a different day altogether!

A little friendly request: ________

Please hold off on calling or texting to check on your dog's pick-up time. We promise we're giving your pup the full VIP treatment, and while we totally get the excitement, constant check-ins can disrupt the "pampered paws" experience for both the dog and the groomer. We're giving your dog the love and attention they deserve, and we'll reach out to you as soon as they're ready for their big debut! Initial please if you're on board with the above!

General Grooming Information: ________

Extra care will be taken when performing grooming procedures; however, you (the owner) must understand that possible reactions such as stress, skin irritation, possible nicks to the skin or a toenail quick may occur. You understand that problems occasionally arise after a grooming visit such as bleeding of nicks, clipper irritation, and mental or physical stress. Grooming can also expose a hidden medical problem or aggravate a current one.

Moles/Skin Lesions/Tumors/Other Irregularities: Because these skin irregularities may protrude from the skin, they are vulnerable to nicks or cuts during the grooming process. Long/sharp dewclaws are sharp and dangerous to staff and other animals and risk getting caught in kennel flooring causing injury. You understand that Senior Pets (age 7+) may have a greater chance of injury. You agree not to hold EDGE Pampered Paws responsible for any injuries that might result from the grooming process.

Emergencies: ________

In case of an emergency, we would like your permission to obtain immediate veterinarian treatment, at your expense, should it become necessary. If your dog's veterinarian is not readily accessible, EDGE Pampered Paws is authorized to use a veterinarian of its choice. EDGE Pampered Paws will not be responsible for any conditions or problems discovered during grooming. Should veterinary care be necessary as a result of any services rendered by EDGE Pampered Paws, EDGE Pampered Paws must be notified within 48 hours of such care. Reimbursement for such care will be at the sole discretion of EDGE Pampered Paws.

Matting: ________

Dogs with severely matted coats require extra attention. EDGE Pampered Paws will not cause serious or undue stress to pets by de-matting. If a dog is severely tangled and/or matted, it poses a greater risk of injury, stress, and trauma. Heavy matting can trap moisture and urine near the pet's skin, allowing mold, fungus, and bacteria to grow and/or reveal skin irritations that existed prior to the grooming process. While all precautions are taken, removing a heavily matted coat can cause nicks, cuts, or abrasions. Aftereffects of mat removal can include itchiness, redness, self-inflicted irritations, or abrasions and -- in extreme cases - failure of hair to regrow. In some cases, pets may also exhibit brief behavioral changes.

Double Coats: ________

EDGE Pampered Paws does not recommend shaving down double-coated dogs unless you deem it absolutely necessary to their health and wellbeing. Some complications that can occur include: the coat not growing back in the same texture and/or color prior to shaving; bald patches; hyper pigmentation of the skin; darkening of the skin where the coat has not grown back, loss of guard coat (the top layer of the coat); coarse re-growth of hair in different directions; exposure to the elements (sun/heat damage or stroke and cold weather); exposure to bug bites; and reduced protection to scratching. Knowing that any and all of the above conditions can occur, you authorize your groomer to shave down your pet and agree not to hold EDGE Pampered Paws responsible for any complications described above which might result.

Aggression: ________

You MUST inform EDGE Pampered Paws if your pet bites, has bitten, or is aggressive to people, other pets, or specific grooming procedures. Muzzles may be used if necessary. Muzzling will not harm your pet and protects both the pet and the groomer. EDGE Pampered Paws reserves the right to refuse/stop services for such pet(s) at any time before, during or after the grooming process should your pet exhibit aggressive behavior. You understand you are liable for any medical care expenses and damages that result from injuries to groomers, staff, and other animals caused by your pet.

Valid Up-to-Date Rabies Vaccine Required: ________

All EDGE Pampered Paws services do require proof of a valid up-to-date rabies vaccine certification (for all dogs and cats over 4 months old in our care). This is for the safety of our staff and the other animals in our care. Exceptions are only given with a signed letter from the pet's medical doctor of exemption.

Fleas/Ticks/Lice: ________

Parasites are a health hazard to your pet as well as to humans. If fleas, ticks, or lice are found on your pet during the grooming process, your pet will be treated with a flea and tick shampoo to kill the parasites and you may be charged an additional fee. Any ticks found will be removed for an additional charge and we strongly encourage you to have your pet tested for Lyme Disease. If lice are found, we will advise you to take your pet to the veterinarian and grooming services will cease immediately as a safeguard against their spread. A $50 flea/tick/lice charge will apply.

No-Shows/Tardiness: ________

We require notification of any cancellation 24-hours prior to your scheduled appointment. No-shows and last-minute cancellations may result in prepayment being required for any future visit. We understand that emergency situations arise and will work with you on a case-by-case basis. Please be respectful of our groomer's time as we are a by-appointment business and another client could have taken your appointment had we known. If you are more than 15 minutes late you risk losing your appointment without notice due to groomer time constraints. A cancellation/no-show fee will be applied at the discretion of EDGE Pampered Paws. Additionally, if you are unable to pick up your dog within a reasonable time frame from the time of pickup phone call request, we will add a tardiness fee (60 minutes beyond phone call).

Photographs: ________

EDGE Pampered Paws may take photos of my pet to use on their website/social media/etc. without further compensation or permission. I understand that any photographs taken are the property of EDGE Pampered Paws.

Release of Claims and Indemnification: ________

THERE IS ALWAYS THE POSSIBILITY THAT AN ACCIDENT COULD OCCUR. THE EQUIPMENT USED IN PROVIDING THE SERVICES IS SHARP, EVEN THOUGH WE USE EXTREME CAUTION AND CARE IN ALL SITUATIONS, POSSIBLE INJURIES COULD OCCUR INCLUDING CUTS, NICKS, SCRATCHES, QUICKING OF NAILS, ETC. YOU HEREBY EXPRESSLY WAIVE AND RELEASE ANY AND ALL CLAIMS WHICH YOU MAY HAVE, OR WHICH YOU MAY HEREAFTER HAVE, WHETHER KNOWN OR UNKNOWN, AGAINST PNW & COMPANY, LLC D/B/A EDGE PAMPERED PAWS, AND ITS SUBSIDIARIES, OFFICERS, DIRECTORS, MANAGERS, EMPLOYEES, AGENTS, AFFILIATES, SHAREHOLDERS, MEMBERS, SUCCESSORS, AND ASSIGNS (COLLECTIVELY, "RELEASEES"), ON ACCOUNT OF INJURY, ILLNESS, DISABILITY, DEATH, OR PROPERTY DAMAGE ARISING OUT OF OR ATTRIBUTABLE TO PARTICIPATION IN THE SERVICES OFFERED BY PNW & COMPANY, LLC D/B/A EDGE PAMPERED PAWS WHETHER ARISING OUT OF THE ORDINARY NEGLIGENCE OF PNW & COMPANY, LLC D/B/A EDGE PAMPERED PAWS OR ANY RELEASEES OR OTHERWISE. YOU COVENANT NOT TO MAKE OR BRING ANY SUCH CLAIM AGAINST THE COMPANY OR ANY OTHER RELEASEE, AND FOREVER RELEASE AND DISCHARGE PNW & COMPANY, LLC D/B/A EDGE PAMPERED PAWS AND ALL OTHER RELEASEES FROM LIABILITY UNDER SUCH CLAIMS. THIS WAIVER AND RELEASE DOES NOT EXTEND TO CLAIMS FOR GROSS NEGLIGENCE, INTENTIONAL OR RECKLESS MISCONDUCT, OR ANY OTHER LIABILITIES THAT NORTH CAROLINA LAW DOES NOT PERMIT TO BE RELEASED BY AGREEMENT. YOU SHALL DEFEND, INDEMNIFY, AND HOLD HARMLESS THE PNW & COMPANY, LLC D/B/A EDGE PAMPERED PAWS AND ALL OTHER RELEASEES AGAINST ANY AND ALL LOSSES, DAMAGES, LIABILITIES, DEFICIENCIES, CLAIMS, ACTIONS, JUDGMENTS, SETTLEMENTS.
